REMOTE PHYSIOLOGIC MONITORING RPM: Remote physiologic monitoring uses connected devices to monitor your patients’ health automatically between office visits.
PRINCIPAL CARE MANAGEMENT PCM: This is often used in conjunction with RPM to help monitor patients with one chronic condition.
CHRONIC CARE MANAGEMENT CCM: This is used for patients with two or more serious chronic illnesses including patients under the care of more than one physician. CCM can also be used in conjunction with RPM.
TRANSITIONAL CARE MANAGEMENT TCM: Helps patients transition from hospital to home by ensuring that a patient contact their physician for a follow-up appt. 7 or 14 days after discharge from the hospital. It also helps patients understand their discharge instructions and ensures they have access to resources if needed.
