Maintaining patients’ connections to their cardiac monitoring devices has always been a challenge. Patients may unplug their devices, travel for a period of time, or, in situations that require manual connection, they may simply become complacent. According to a 2021 study published in Circulation: Arrhythmia and Electrophysiology, around 50-60% of patients with CIEDs in the U.S. had their devices connected to remote monitoring systems.
At Cardiac RMS, we are proud that our patient connection rate averages 96%!
Our Patient Engagement Services Program provides comprehensive services to initiate and maintain device connectivity with your patients. We can provide you with as little or as much support as you need.
We are happy to enroll patients, walking them through the equipment set up and answering their many questions. We can continually monitor and maintain the device manufacturer sites to ensure regular transmissions and can clean up the vendor sites when needed. We are also happy to schedule patients for their 31- and 61-day services. Most importantly, we will call patients repeatedly and make every attempt to keep them connected.
PES will call and/or text the patient to inform them their monitor/app is disconnected and provide the vendor’s phone number for the patient to follow up. Outreach attempts will be documented in the vendor site/software.
PES will continue outreach attempts for 3 consecutive weeks. If the patient remains disconnected, PES will reach out monthly for up to 6 months. After this period, the patient will be marked as archived/non-compliant in the vendor site/software.
PES will perform light troubleshooting of the monitor with the patient.
PES will connect patients with the vendor and stay on the line to provide additional troubleshooting support as needed.
PES will work within vendor sites to schedule all devices on either 31- or 91-day schedules.
PES will call or text reminders for patients to send transmissions, focusing on inductive patient populations.
PES will enroll patients in vendor sites.
PES will manage the transfer of patients in and out of vendor sites.
PES will work with clinics to obtain high priority transmissions (same day, Monday – Friday).
